Digital health startup Suvera has teamed up with NHS South East London Integrated Care System (ICS) to launch a ground-breaking program aimed at improving hypertension control in the borough of Lewisham.
The six-month initiative will tackle health disparities and enhance patient outcomes by leveraging Suvera’s innovative virtual care solutions. With a strong focus on improving hypertension management in Lewisham, Suvera’s team of clinical pharmacists will implement proactive care strategies, risk prioritization, and remote blood pressure monitoring to address critical health issues.
This partnership aims to address health inequalities related to Cardiovascular Disease, particularly in Lewisham where hypertension control rates are low. By targeting patients in need, the program aims to increase hypertension control to meet national targets, reducing the risk of heart disease and stroke.
Suvera, a participant in the NHS Innovation Accelerator program, will provide a hybrid virtual service with independent prescribing clinicians conducting virtual reviews and offering comprehensive care management. The service will include multilingual engagement tools and accessibility features, with a focus on getting diagnosed patients to their target blood pressure level to prevent strokes and save lives.
