By utilizing the Patient Need Groups (PNGs) segmentation tool in the Johns Hopkins Adjusted Clinical Groups (ACG) System, the healthcare team is able to identify and prioritize the most complex patients, thus optimizing their health before the onset of the winter season. This approach has also allowed the centre to complete 70% of QOF reviews by September 2023, a significant improvement from the previous year. Dr. Kumar stated that by utilizing this digital approach, they have been able to efficiently allocate the right healthcare professional to the right patient at the right time, ultimately maximizing the use of personnel and reducing the need for repeated appointments.
