With GUIDE Deadline Near, Hospice Providers See New Possibilities for Dementia Care

Without the support of family caregivers, many hospice and palliative care patients would be unable to receive care at home. Emerging payment models, such as the Guiding an Improved Dementia Experience (GUIDE) model unveiled by the U.S. Centers for Medicare & Medicaid Services (CMS) in July 2023, are now including caregiver support as a key component.

Providers interested in participating in the GUIDE model have until Jan. 30 to submit applications for this eight-year payment demonstration. The model aims to improve the quality of life for dementia patients and their caregivers by addressing behavioral health and functional needs, and coordinating care and improving care transitions between community, hospital, and post-acute settings.

The rise of dementia-related illnesses among hospice patients has prompted the need for more comprehensive caregiver support. The GUIDE model could drive improvement in this area, providing up to $2,500 in annual reimbursement for caregiver respite services.

However, many hospices face financial obstacles in expanding and diversifying caregiving resources in serious illness and end-of-life care. Most providers lack the infrastructure for caregiver support, as required by CMS. The GUIDE model presents an opportunity to strengthen care management for dementia patients, creating a more comprehensive support system for patients and caregivers.

Areas of Need and Opportunities

Caregivers of patients with dementia face significant challenges in care navigation, and often require support in addressing medication, transportation, and mental health needs. The financial resources from the GUIDE model could help hospices build out interdisciplinary staff and provide additional support services for caregivers, ultimately improving patient care and outcomes.

Empowering family caregivers with adequate resources and training is essential for meeting the complex and long-term needs of dementia patients. Creating a sustainable system for caregiver support is critical to ensuring high-quality care at home.
