OneLondon’s UCP expands to sickle cell disease patients

OneLondon’s Universal Care Plan (UCP) now includes support for sickle cell disease patients, integrated with the NHS National Record Locator (NRL) and the NHS App.

Originally focused on end-of-life care, OneLondon’s UHP platform, powered by Better, expanded its services to include individuals with sickle cell disease in March 2024.

Effective management of sickle cell disease requires comprehensive care planning and timely interventions for symptoms such as anemia and sickle cell crisis.

With approximately 60% of diagnosed individuals receiving treatment in London, the UCP now supports 5,000 people with the disease on its integrated care platform.

The recent integration with the NHS App allows patients to access their care plans on the go, facilitating seamless care coordination across healthcare settings.

By leveraging technology and a person-centered approach, the UCP aims to set new standards in integrated healthcare solutions, continuously evolving to meet the changing needs of patients in London.

Future plans for the UCP include expanding services to include cohorts such as dementia, frailty, children, and young people, emphasizing its commitment to advancing person-centered care initiatives.

Since its deployment in August 2022, the UCP has created over 24,000 new care plans by September 2023, showcasing its impact on improving care delivery in the region.
