Curana Health and the Serviam Care Network have joined forces to launch a new Value-Based Care Alliance in Florida. This collaboration aims to extend value-based care services to senior living communities in the state, with a focus on enhancing care outcomes in alignment with Medicare Advantage plans.
The partnership, which originally began in Virginia and now includes six senior living operators across 23 communities, is expanding into Florida in a similar fashion. The goal is to provide support for operators and residents in both primary care and technology, allowing older adults to age in place.
With two operators already enrolled in the Florida alliance and more expected to join in the coming months, Curana Health brings a wealth of experience in the Florida market through its Medicare Advantage plan and Curana Health Medical Group. The alliance will also work in accordance with the Centers for Medicare and Medicaid GUIDE model to enhance dementia care and caregiver support.
By leveraging their resources and scale, the Curana-Serviam partnership aims to overcome the challenges that many senior living providers face in advancing value-based care growth. Communication, information sharing, and continual iteration are key components in creating a valuable model for all operators and residents involved.
