An actuarial report conducted by Strategia has affirmed the effectiveness of Frimley Health’s remote monitoring programme for patients with multiple chronic conditions and frailty.
Since its inception in 2022, Frimley ICS has successfully enrolled over 8,000 patients in the program, utilizing remote monitoring technology from Graphnet to detect and manage deteriorating health conditions early on.
An analysis of the initial 3,673 patients by Strategia revealed a significant reduction in A&E visits, hospital admissions, and outpatient appointments between December 2022 and October 2023.
Strategia’s review predicts annual savings of £5-£8 million from these measures and also noted decreases in GP visits, emergency calls, and prescription issuance for monitored patients.
Sam Burrows, NHS Frimley’s chief transformation officer, praised the program for providing timely care and reassurance to patients and professionals alike, while emphasizing the importance of spreading these beneficial practices across the NHS.
Frimley Health and Care collaborated with Graphnet to develop population tools for remote monitoring, enabling patients to share health data and receive advice and interventions via the DOC@HOME app.

Connected Care, Frimley’s digital transformation initiative supported by Graphnet Health, initially launched in care homes and has now expanded to patients with complex needs, showcasing the program’s ongoing success and potential for growth.
